Teaching Application

Multi-line address

Teaching artists who reside outside Union County:

Please apply! We'd love to have you. Let us know of your needs in traveling to and from Creston Arts.
I would like to be reimbursed for the round-trip mileage for each class session (at the 2023 federal rate of $0.655 per mile, calculated using the address on this application form). I understand that my receiving mileage reimbursement will increase the cost of class for my students.
I do not need to be reminbursed for mileage.
Do you need housing accomodations?
Yes
No

What is especially interesting about this class?

What art medium(s), topics, skills, etc., will students explore in this class?

What work and artistic experience and/or academic degrees support your teaching the class you’re proposing here? Have you taught this content before, and for how long? Note: your bio should be written in the third person. For example, “Maria is a watercolor painter”, not “I am a watercolor painter”. Creston Arts may edit your bio for length, clarity, and style.

Supplies
I want Creston Arts to purchase supplies (note: they remain the property of Creston Arts after the class).
I want to be reimbursed for supplies (note: receipts will need to be provided).
I want Creston Arts to provide some supplies, I will provide others.
Students will be instructed on supplies to bring.
